QUESTION:

What happens if, in your last semester, you don't meet the degree requirement to graduate and you've already filled out an Intent to Graduate form? My GPA for my program is fine but the average needed for the faculty I'm in has just dropped by 3% (based on my calculation) due to the electives I took last semester. Would I be unable to graduate?

Your advisor/program director only reviews whether you've met graduation requirements AFTER your grades for your last semester are available. So, around now, they should be reviewing them. If they find you haven't met the requirements, they will notify the Registrar's Office, who will then get in touch with you to deliver the bad news that you aren't eligible to graduate and explain what your options are. (In your case, taking another course/term to raise your average.)
